TERMS
By purchasing or subscribing to our membership plans, you (“Member”) have read and agreed to the following terms and conditions (“Membership Agreement”) by Millennia Wellness MD, PLLC (“Millennia Wellness MD” or the “Company”).
BENEFITS
Membership benefits include discounts on services and products as described on the Membership page of our website. Services are provided on a “use it or forfeit” basis.
TERM
Membership shall commence on the date of purchase or subscription and will continue for a period of twelve (12) months (“Membership Term”). This agreement shall automatically renew for successive one-month periods (“Renewal Term(s)”) unless either party provides written notice of non-renewal at least thirty (30) days prior to the end of the then-current Membership Term.
MEMBERSHIP FEE
In consideration for the services provided by the Company, the Member agrees to pay the Company the monthly membership fee of $150, payable in advance on the same day of each month.
The Company reserves the right to modify the membership fees upon sixty (60) days prior written notice to the Member. The Member has the right to cancel the membership upon notice of changes to the membership fees by providing the Company a thirty (30) day written notice. Otherwise, membership is automatically renewed at the new membership fee.
PAYMENT TERMS
Payment for the membership fees shall be made by the Member via credit card accepted by the Company. In the event of late payment, the Company reserves the right to suspend or terminate the Member’s access to the services provided under this Agreement until payment is received in full.
BENEFICIARY
Membership benefits are solely tied to the Member and cannot be transferred or shared with any other person(s).
TERMINATION
Should the Member decide to terminate the membership before the end of the Membership Term, the Member agrees to pay the Company a one-time termination fee of $150.
The Company may terminate this Agreement immediately if the Member breaches any provision of this Agreement or engages in any conduct that the Company, in its sole discretion, deems to be detrimental to the Company or its other members.
GOVERNING LAW
This Agreement shall be governed by and construed in accordance with the laws of Texas where Millennia Wellness MD, PLLC operates.
